Extended Registration Form & Liability Waiver

For some tournaments, organizers may require you to fill out additional information in the extended player registration form. This form may include a waiver of liability.

How to fill out the extended player registration form:

Attention! If you do not sign the waiver of liability, the organizer has the right to refuse your admission to the tournament.

  1. The team manager will send you a link to the player registration form. You will also receive an email asking you to fill out the form.

  2. Fill in all the fields added by the organizer. This may include National Federation ID, WFDF ID, and more.

  3. If there is a waiver of liability in the form, read it carefully and check the box that you agree with it.

  4. Click "Submit".

If you are under 18 and there is a waiver of liability in the player registration form:

Attention! Please provide the real contact information for your parent or legal guardian. If your parent or guardian does not sign the waiver of liability on your behalf, you will not be allowed to participate in the tournament.

  1. The team manager will send you a link to the player registration form. You will also receive an email asking you to fill out the form.

  2. Fill in the details of your parent or guardian: first and last name, and email.

  3. Fill in the additional fields added by the organizer. This may include your graduation year or National Federation ID.

  4. Click "Submit".

  5. An email will be sent to your parent/guardian address asking them to sign the waiver of liability for you.

  1. Go to your personal profile by clicking on your name in the upper right corner, or on the user icon in the mobile version of the site.

  2. Click on "Profile".

  3. In the list of your upcoming events there will be a link to the player registration form / waiver of liability.
