Extended Player Registration

You can add custom fields to the player registration form

Step 1. Open Settings

From the main menu, click Settings.

Step 2. Open Extended Player Registration Form

Scroll down to the Custom settings section and click Extended Player Registration Form.

Step 3. Customize the Form

Toggle the Liability Waiver on or off depending on your needs. You can also add extra questions to the registration form. Available question types include:

Single line text
Multi-line text
Checkboxes
Dropdown menu
Radio buttons

After editing the form, click Save to apply your changes.

The registration form already has the following fields pre-filled: name, team, gender, age, email. You do not need to duplicate these fields.

How to export data to CSV:

Go to the "Approved Players" section.
Click the "Download CSV" button.
For ease of use, you can upload this file to Google Sheets or similar software and edit it as you see fit.
