# Complete event forms and waivers

Complete the player survey and the age-appropriate waiver required for an event.

An event may require a player survey, a waiver, both, or neither. Complete each action shown for the player before the organizer's deadline.

## Complete the player survey

1. Open the required action from the event email or your global profile.
2. Select the applicable player.
3. Complete the fields added by the organizer.
4. Review your answers and submit the survey.

For a managed child, the Family manager opens the action from the child's profile and completes the survey.

For a Hat event, you can also open **Manage Application** from the event page.

## Sign the waiver

The signing route shown for the player depends on the applicable profile-management and authority route:

* **Adult route:** a player who is 18 or older reviews and signs the adult waiver in their own account.
* **Family-managed route:** the linked Family manager reviews and signs the minor waiver for the managed child.
* **External-adult route:** for another player under 18, Ultie sends a single-use external link to an authorized adult. The adult can review and sign without creating an Ultie account.

## What happens next

Survey and waiver statuses update independently after each action is completed.

## If something blocks you

* **No action appears:** check that the player is on the event roster, then contact the team manager or organizer.
* **The adult signing link no longer works:** request a new link; resending deactivates the previous one.
* **Ultie cannot select the correct waiver:** check that the player's birthday is present and correct.
