# Set up player surveys & waivers

Add player questions, configure adult and minor waivers, and choose when each group must sign again.

## Open the setup

Use either route:

* **Settings → Custom settings → Player Survey & Waiver**
* **Teams & Rosters → Player Survey & Waiver**

The base player form already collects first name, last name, Gender, team, and birthday. Do not add the same questions again.

## Add player survey questions

1. Select **Add question**.
2. Choose **Single line**, **Multi line**, **Checkboxes**, **Dropdown**, or **Radio**.
3. Enter the question and any options required by the selected type.
4. Turn on the required-field setting when every assigned player must answer it.
5. Add or delete questions as needed.
6. Select **Save**.

Use the share link shown on the page when you need to send the player survey & waiver entry point directly.

## Add adult and minor waivers

1. Turn on **Liability Waiver**.
2. Open the adult waiver editor, enter the text, and save it.
3. Open the minor waiver editor, enter the text, and save it.

## Decide whether participants must sign again

After changing a waiver, use its **Send for re-signing** toggle if that group must accept the new version.

## Who completes the player requirements

* Adults complete their own player survey and sign the adult waiver.
* A Family manager completes the player survey and signs the minor waiver when the player's profile uses the Family-managed route.
* Every other player under 18 uses the external-adult route for the minor waiver; the authorized adult does not need an Ultie account.
